The Children's Home of Wyoming Conference partners with children, families and communities throughout New York State to inspire hope, develop skills and cultivate healthy relationships for positive futures. $21.50-22.50 an hour Serving Cortland and Tompkins counties Position Summary: Health Home Care Managers at the Children's Home provide strength-based, youth-focused services to help eligible children access medical and behavioral healthcare, social services, education, housing, and other essential supports. Working in partnership with the child, their family, and service providers, they coordinate the most appropriate resources to meet the child's needs, improve well-being, and support independence in the least restrictive environment. Comprehensive Care Management uses a team-based approach to create a single care plan that ensures appropriate, high-quality, and cost-effective support. It also provides oversight and coordination to make sure health and social services are delivered effectively.
