About The Position

The Health Home Plus Care Manager position at Lakeview Health Services involves providing intensive support, advocacy, and coordination of services for individuals with mental illness and chronic health conditions. The role requires working from a trauma-informed care perspective to engage individuals who may be disengaged from care, ensuring they receive the necessary support and resources to achieve their personal goals. The position is supervised by the Care Management Program Manager and involves maintaining a caseload of up to 20 individuals, conducting assessments, and collaborating with various service providers to ensure comprehensive care.

Requirements

  • A Master’s degree in a qualifying field and one year of experience; OR
  • A Bachelor’s degree in a qualifying field and two years of experience; OR
  • A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two years of experience; OR
  • A Bachelor’s degree or higher in any field with three years of experience, or two years as a Health Home care manager.
  • NYS Driver’s License.

Nice To Haves

  • Experience providing direct services to people with Serious Mental Illness or developmental disabilities.
  • Knowledge of community services within the service area.
  • Understanding of motivational interviewing and trauma-sensitive perspectives.

Responsibilities

  • Maintain engagement with individuals who are often disengaged from care.
  • Support individuals through skills and practices including motivational interviewing and trauma-informed care.
  • Hold a caseload of up to 20 individuals who meet Health Home eligibility.
  • Ensure documentation is accurate and completed on time.
  • Conduct comprehensive assessments and coordinate services using evidence-based practices.
  • Demonstrate proficiency at navigating the health care system.
  • Develop and revise individual plans of care consistent with Health Home requirements.
  • Monitor consumer wellness and ensure well-coordinated care among all providers.
  • Attend necessary meetings and maintain regular communication with supervisors and service providers.
  • Participate in on-call rotation and adhere to billing standards.
