About The Position

The Health Home Plus Care Manager works under the general supervision of the Care Management Program Manager, providing intensive support, advocacy, linkage, and coordination of services for individuals with mental illness and/or chronic health conditions. The role emphasizes a trauma-informed care perspective and aims to create a healing environment that respects the perspectives and experiences of individuals, families, staff, and communities. The Care Manager will engage with individuals who are often disengaged from care and support them through various skills and practices, ensuring comprehensive assessment and coordination of care.

Requirements

  • Master’s degree in a qualifying field and one year of experience; OR
  • Bachelor’s degree in a qualifying field and two years of experience; OR
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two years of experience; OR
  • Bachelor’s degree or higher in any field with three years of experience, or two years as a Health Home care manager.
  • NYS Driver’s License.

Nice To Haves

  • Experience providing direct services to people with Serious Mental Illness or developmental disabilities.
  • Knowledge of community services and mental health issues.
  • Skills in motivational interviewing and trauma-informed care.

Responsibilities

  • Maintain engagement with individuals who are often disengaged from care.
  • Support individuals through skills including motivational interviewing and trauma-informed care.
  • Hold a caseload of up to 20 individuals who meet Health Home eligibility.
  • Ensure accurate and timely documentation.
  • Conduct comprehensive assessments and coordinate services using evidence-based practices.
  • Navigate the health care system and make referrals to necessary services.
  • Develop and revise individual plans of care consistent with Health Home requirements.
  • Monitor consumer wellness and ensure well-coordinated care among all providers.
  • Attend necessary meetings and maintain regular communication with supervisors and service providers.
  • Participate in on-call rotation and adhere to billing standards.
