Responsibilities

• Provide ongoing, community-based support for an assigned caseload of health plan enrollees.
• Establish relationships with enrollees, care coordination teams, and providers.
• Conduct new enrollee outreach and orientation, arranging appointments and transportation as needed.
• Assist enrollees in navigating the healthcare delivery system and community resources.
• Administer Health and Wellness Questionnaires and gather information for continuity of care.
• Work with Peer Support Specialists and leverage community services for enrollee support.
• Ensure enrollees access care and receive necessary services.
• Monitor enrollee progress according to the Care Coordination Plan and remind them of self-management tools.
• Engage the Care Coordination Team if enrollees have difficulties adhering to their care plans.
• Maintain up-to-date documentation in the Care Coordination Plan and prepare information for team meetings.
