About The Position

South Country Health Alliance has an immediate need for a Health Equity and Population Health Coordinator! Located in Medford, MN, South Country Health Alliance is dedicated to improving the health and well-being of Minnesota Health Care Program enrollees living in rural Minnesota communities. The Health Equity and Population Health Coordinator is responsible for the coordination, communication, implementation, and reporting of South Country's health equity, social determinants of health, and population health management programs and projects. This includes initiatives required by state and federal regulations and contracts. The position ensures that initiatives are designed to achieve the highest standards of quality of care and positive health outcomes for members. The Coordinator will be accountable for coordinating project teams and collaborating with internal and external stakeholders, meeting project objectives, and reporting to regulatory entities and internal staff on project status and outcomes. The role requires a Bachelor's degree in health care administration, public health, business administration, social science, or a related field, along with at least 3 years of previous work experience in a managed care organization, public health, project management, quality improvement, or related work. Experience with government programs, population health management (PHM), health equity (HE), or social determinants of health (SDOH) is preferred. Intermediate expertise with Microsoft Office Suite is also preferred. The Coordinator will be responsible for coordinating work related to National Committee of Quality Assurance (NCQA) Health Equity Accreditation and population health management, monitoring compliance with ongoing state and federal requirements, and assisting with policies and procedures. The position involves organizing the planning, implementation, and evaluation of improvement initiatives employing best practices that support initiatives addressing health equity, social determinants of health, and population health management to improve health outcomes. The Coordinator will compile and organize business requirements to assure quality improvement initiatives are being met, develop and maintain partnerships with community groups, collect and analyze health care-related information, conduct needs assessments, and collaborate across departments to support information sharing and reports. Additionally, the Coordinator will develop and facilitate training on social determinants of health, health equity, and cultural diversity and inclusion to promote health improvement strategies.
