Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Health Educator team! The Health Educator works independently and as part of a team to support internal and external community health programs.

Requirements

  • Associate's degree in Community Health or Health Education (or equivalent of education and directly related experience).
  • One or more years working in a community health setting preferred.
  • Experience conducting individual or group education presentations.
  • Strong Interpersonal and oral communication skills.
  • Ability to communicate effectively in both English and Spanish required.
  • Demonstrated cultural competency to work in a low income, minority environment preferred.

Responsibilities

  • Conduct educational appointments with patients, prepare for and facilitate group sessions, providing patient follow-up and presenting educational workshops that focus on nutrition, diabetes, disease prevention and physical activity.
  • Provides training and assistance for staff regarding health education issues.
  • Acts as member of a patient care team with clinical staff and other health care providers.
  • Strong oral communication skills and can communicate effectively with patients, health care providers and community members.
