About The Position

The incumbent shall help slow the spread of HIV through prevention efforts such as education, counseling, testing, contact interviewing, The incumbent will perform HIV pre and posttest counseling throughout the Alachua County Health Department system and other clinics as assigned, according to CDC and FDOH guidelines. Elicits information to coordinate investigations of other persons at risk. Ensure all clients are linked to care as needed. The incumbent will give support, comfort, and care to those living with HIV and STDs whenever possible. The incumbent will complete all electronical medical record documentation properly (client registration, demographics, 1628, lab slip, progress notes, etc.) Scan documents into electronic medical records as needed. Completes reports as assigned. Maintain appropriate data base logs, records, and documents to prepare timely reports. Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity, and appropriate access, for all information, both confidential and public record, this is stored in hard copy or electronic formats The incumbent will serve as a PrEP co-coordinator and will promote, educate, counsel, and link clients to PrEP services as needed. incumbent will complete intake services for referred PrEP clients, schedule PrEP medical appointments, complete and assist clients with patient assistant programs, coordinate and act as a liaison for pharmacy assistance programs, and routinely follow up with PrEP clients. incumbent will follow up and complete PrEP care coordination requests. incumbent will serve as a liaison for statewide and local injectable PrEP programs. The incumbent will assist in the support, planning, coordination, and attendance in community outreach and engagement events/activities. The incumbent will collaborate with other prevention staff and community partners to support the outreach for our communities and those living with HIV and STD. Will conduct rapid Testing for Proof of Status as needed. Attends all required staff meetings and trainings. Performs other related duties assigned.

Requirements

  • Possess a valid driver’s license.
  • Intermediate Computer skills including Microsoft Programs.
  • Knowledge or experience working with HIV or STD Programs.
  • Knowing how to find information and identifying essential information.
  • Talking to others to effectively convey information
  • Communicating effectively with others in writing as indicated by the needs of the audience
  • Knowledge of methods of compiling, organizing and analyzing data for the purpose of monitoring and evaluating.
