The Health Educator is responsible for collaborating with patients, family members, health-care team members, health-care team members from other organizations, and other stakeholders to reduce or eliminate barriers to care. This role involves building and maintaining relationships with patients and other stakeholders to improve the quality of care, particularly for those with chronic illnesses, histories of avoidable emergency department visits, prenatal needs, and other socially mediated health concerns. Health Educators engage with patients through various mechanisms, including face-to-face contacts, leading classes/groups, and community functions. They assist patients in understanding and managing their health care plans, provide educational materials, and facilitate communication between patients and health-care teams. Additionally, they participate in meetings and trainings to evaluate the effectiveness of their functions and complete necessary reporting and documentation.
