About The Position

Your main role is ensuring every patient gets uninterrupted care from our healthcare team. Coordinate patient care seamlessly across different stages. Work with providers to achieve our priorities.

Requirements

• Possess a high school diploma, GED, or equivalent education.
• Have under (1) one year of relevant professional experience.

Responsibilities

• Review reports to find patients needing adult preventative, diabetes, and other clinical services.
• Use patient records to create reports for care coordination.
• Work with various teams and resources to coordinate care.
• Help find community resources for those in need.
• Interact with patients, recommend services, schedule appointments, and assist with paperwork.
• Resolve concerns for patients and families.
• Provide approved education and telephonic engagement to ensure adherence to care plans.
• Monitor patients moving between hospital, home, and their primary care providers.
• Perform follow-up tasks and update care providers on critical issues.
