About The Position
The Health Coach plays a vital role in the Complex Care Management and Primary Care teams, focusing on promoting disease management, preventative care, and wellness. This position encourages patients to actively participate in their health journey by providing essential tools for making healthy lifestyle choices and adopting long-term healthy behaviors. The Health Coach collaborates with the Complex Care Manager to coordinate and monitor patient care, ensuring alignment with comprehensive care plans and goals, while also promoting quality care through effective communication with patients, families, and the care team.
