Alanté Health is seeking a dedicated HEALTH COACH to support patients with chronic conditions such as diabetes, hypertension, COPD, CKD, and heart failure by providing education, coaching, and ongoing support. This role empowers patients to take charge of their health through behavior change, self-monitoring, and collaboration with the interdisciplinary care team. Requirements

  • High School Diploma or GED.
  • Two years of experience in health coaching, wellness, or chronic condition support.

Nice To Haves

  • Bachelor’s degree in Health Education, Nutrition, Exercise Science, Public Health, or a related field preferred.
  • Certification as a Health Coach (CHC, NBC-HWC, or equivalent) preferred.

Responsibilities

  • Provide patient outreach via phone, video, and in-person coaching sessions.
  • Support lifestyle changes including nutrition, exercise, stress management, and smoking cessation.
  • Teach patients the use of self-monitoring tools such as blood pressure cuffs, glucose meters, and weight scales.
  • Assist patients in setting realistic, individualized health goals and track progress over time.
  • Document coaching interactions in the electronic medical record (EMR).
  • Collaborate closely with nursing staff and care coordinators to ensure continuity of care.
  • Performs other duties as assigned.
