Your responsibilities will include developing personalized health plans, providing guidance on lifestyle modifications, treatment adherence, navigating systems of care, and fostering a supportive environment for clients to achieve their health goals

Requirements

  • Education: High school degree/GED, unless degree specific in funding source.
  • Work Experience: Experience in health coaching, wellness counseling, or a related role.
  • Experience working with diverse populations and addressing various health needs.
  • Experience working in healthcare and social service systems.
  • Previous roles involving assessment, planning, and advocacy for patients.
  • Technical Experience: Ability to learn and manage multiple databases.
  • Soft Skills: Strong interpersonal and communication skills.
  • Ability to inspire and motivate clients to make positive changes in their health behaviors.
  • Empathy and understanding of diverse cultural perspectives.
  • Ability to work independently and collaboratively within a team.
  • Problem-solving and decision-making abilities.
  • Commitment to upholding ethical standards and confidentiality.

Responsibilities

  • Conduct comprehensive health assessments to understand clients' needs, goals, and challenges.
  • Develop individualized health plans tailored to clients' unique needs and circumstances.
  • Provide guidance on treatment adherence, nutrition, exercise, stress management, navigating systems of care, and other health-related areas.
  • Build strong relationships with clients, fostering a supportive and motivating environment.
  • Regularly communicate with clients to monitor progress, address concerns, and provide encouragement.
  • Offer educational resources and information to enhance clients' understanding of HIV, other health concerns and wellness.
  • Conduct one-on-one or group sessions to share knowledge on healthy living practices.
  • Maintain accurate and up-to-date client records in compliance with organizational standards.
  • Adjust health plans as needed to ensure alignment with clients' evolving needs.
  • Collaborate with healthcare providers, nutritionists, and other professionals to create holistic health plans.
  • Provide HIV disease prevention and treatment adherence education.
  • Adhere to all agency policies, including but not limited to Confidentiality, Employee Handbook, Health Care Corporate Compliance Plan, Standards of Conduct, training, and other relevant policies.
