AccessHealth is a program to assist qualifying low-income adults without health insurance in Anderson County. AccessHealth brings together a network of healthcare providers and community agencies to ensure that those in underserved communities have access to high quality of care in the right place at the right time. The role of the Health Coach is to establish and maintain collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. The Health Coach is to conduct outreach to specific populations as identified by the Healthy Outcomes Plan or other population health initiatives; implement strategies to promote, maintain and improve individual health and defined or targeted outcomes. The Health Coach provides care management in a variety of settings, including inpatient/bedside, medical homes/PCP, community, video visits and/or home visits. Care management will include a variety of issues, both clinical and nonclinical.
