About The Position

The Health Coach at Karoo Health plays a crucial role in providing health coaching and coordination for members with cardiovascular diseases. This position focuses on improving patient outcomes through education, support, and the implementation of personalized care plans. The Health Coach will work closely with a multidisciplinary team to ensure patients receive comprehensive care and support in managing their health.

Requirements

• Bachelor's degree in Health Education, Exercise Physiology, Health Sciences, or Human Services.
• Minimum of 3 years of related experience in health education, health coaching, or patient education.
• Certification or advanced certification in health and wellness coaching (e.g., National Board of Health and Wellness Coaching).
• Experience with triage and working within a team-based care model in an onsite or virtual setting.
• Strong interpersonal skills and ability to quickly establish rapport with patients over the phone.
• Ability to inspire patient motivation and enthusiasm for change and follow-through on care plans and goals.
• Comfortable with ambiguity and a fast-paced culture, with a solution-oriented mindset.
• Ability to utilize data to make informed decisions.

Responsibilities

• Determine patient eligibility for Karoo Health wraparound services.
• Operate within practice settings and coordinate across multiple team members.
• Conduct initial patient onboarding and activation through program and technology education.
• Complete ongoing care plan education and monitor adherence to the care plan.
• Coordinate health coaching as directed by the Cardiologist or other qualified health professionals.
• Implement a coaching plan to activate the patient and address barriers to change.
• Coordinate with the Cardiology practice to provide insight into the patient's journey and clinical issues.
• Monitor incoming clinical submissions from wearable devices.
• Regularly interact with practice staff to provide insights into patient admissions, readmissions, and ED visits.
• Engage in special projects, initiatives, and other job duties as assigned.
