About The Position

The Health Coach position within the Healthy Outcomes Team at CVS Health focuses on advocating for the Medicare population by facilitating access to essential health services and resources. Requirements

• 1-3 years of experience with Microsoft Office.
• 3-5 years of practice experience in Social Work, Gerontology, Nursing, or Population Health.
• Experience in Managed Care or Medicare.

Nice To Haves

• Healthcare licensure and/or certifications (RN, MSW, LCSW, etc.).
• Bachelor's degree or equivalent experience.
• Bilingual (Spanish).
• Previous telephonic coaching, education, or case management experience.
• Experience with behavioral change and/or motivational interviewing.

Responsibilities

• Conduct inbound and outbound calls to members and providers to facilitate healthcare services such as visits, screenings, vaccinations, and medication refills.
• Assess members using motivational interviewing skills and other tools to evaluate their health status and identify barriers to care.
• Partner with members and their providers to integrate available internal and external programs or resources.
• Utilize active listening skills to provide coaching, information, and support, empowering members to make independent healthcare choices.
• Interpret guidelines and criteria to positively impact members' health.
• Provide relevant healthcare information to enhance members' understanding of their health status.
• Encourage member participation and engagement in healthcare decisions.
• Implement strategies to reduce risk factors and overcome barriers related to complex health conditions and social determinants.
• Develop proactive action plans to improve both short- and long-term health outcomes.
