About The Position

The Health Coach RN provides lifestyle health coaching with a focus on behavior modification to assist members in managing their health and achieving a better quality of life. This role involves planning and supporting members in reaching their health goals through education, experience, and judgment, while working under general supervision.

Requirements

• Current Louisiana State RN License
• 2 years of related experience in health coaching or similar roles
• Strong communication skills, both written and verbal
• Experience in weight loss and diabetes care
• Ability to develop and implement care plans

Nice To Haves

• Experience with obesity and hyperlipidemia management
• Familiarity with wellness program development
• Skills in creating educational content for health coaching

Responsibilities

• Assist health plan members in setting realistic holistic health goals using system tools and judgment.
• Build trust with individuals to help them meet or exceed health targets, particularly for those with disease-based risks or high utilization.
• Measure and communicate individual goal status, documenting progress and improvements using system tools.
• Coordinate care teams to remove obstacles to health goals, including scheduling appointments and ensuring prescriptions are filled.
• Participate in member Healthy Lives screenings to obtain and document biometric data appropriately.
• Communicate effectively with providers to ensure alignment of health coaching care plans with treatment outcomes.
• Assist in worksite assessments, evaluating policies related to health and wellness.
• Produce and participate in wellness webinars, newsletters, and other special projects as needed.
• Document progress towards goals in the health coaching tool for employer reports on wellness screening compliance.
• Estimate cost savings for employers based on individual weight loss and health coaching outcomes.
