About The Position

Island Pond Health Center, part of Northern Counties Health Care, is seeking a dedicated and skilled Registered Nurse (RN) to join our team. This on-site position plays a crucial role in patient care, working closely with providers and other healthcare professionals to ensure high-quality services.

Requirements

  • Valid Registered Nurse (RN) license in Vermont.
  • Graduate of an accredited RN program.
  • Minimum one year of nursing experience in a medical office, community health center, or similar setting.
  • CPR/BLS certification required (ACLS and PALS may be required at some sites).
  • Strong clinical judgment, problem-solving skills, and the ability to multitask.
  • Proficiency in EMR systems and computer applications.
  • Ability to handle physical demands, including lifting up to 50 lbs and working under emergency conditions.

Responsibilities

  • Screen and assess patient phone calls for providers.
  • Perform clinical evaluations using knowledge of disease processes and treatments.
  • Assist with walk-in and call-in appointment triage.
  • Coordinate patient flow and provider scheduling for efficient operations.
  • Carry out standing orders for medication refills, tests, and routine health maintenance.
  • Conduct nursing assessments and preliminary workups.
  • Take and record vital signs (temperature, pulse, blood pressure, etc.).
  • Administer medications, vaccines, allergy injections, and IVs per protocol.
  • Assist with minor procedures, including wound care and suturing support.
  • Perform EKGs, vision, and audiometry screenings.
  • Provide education on treatment plans, medications, and health maintenance.
  • Participate in community health education programs.
  • Perform in-office lab tests and document results.
  • Draw and prepare specimens for testing.
  • Dispense pharmaceuticals per provider orders and monitor inventory.
  • Maintain accurate patient records in the Electronic Medical Record (EMR).
  • Participate in quality improvement initiatives and infection control measures.
