The Kansas Statewide Farmworker Health Program (KSFHP) is looking for a Grant Monitor to manage grant funding for these activities and to supervise a passionate and dedicated team of care coordinators to connect clients to healthcare. Grant objectives emphasize preventative care for chronic conditions, including diabetes, hypertension and behavioral health and improved access to healthcare for vulnerable farmworkers. Successful candidates will have 5 years of experience in planning, implementing and monitoring program activities. Ability to communicate and have attention to detail are necessary skills for this position. Experience supervising staff and managing teams is preferred. The Health Center Grant Monitor position (K0222345) will serve as Grant Monitor for a $2.4M Health Center Grant, overseeing applications, budgets, deliverables, and implementation of objectives focused on population health. Manage grant activities to ensure compliance with federal and state regulations. Monitor contractor performance, review fiscal and program reports, and oversee data collection and evaluation of healthcare measures. Collaborate with Bureau staff, providers, and partners to execute work plan strategies, and represent the program at state, regional, and national conferences. Regularly monitor and manage grant activities, deadlines, and budgets to comply with federal and state funding and procurement statutes and regulations. Design and manage review processes for budget/fiscal reports and work plan deliverables for program auditing/compliance purposes. Oversee the collection of grant-specific data collection and progress toward defined healthcare measures for evaluation activities. Supervises the Health Center Billing Specialist and Administrative Assistant responsible for processing vouchers for approximately 200 healthcare providers. Supervises two Analyst positions responsible for data collection, policy and procedure development, and quality initiatives. Supervises at least four Case Managers and provides direct program management for at least four contracted Health Promoters responsible for conducting statewide client and provider outreach. Maintains outreach plan and initiatives to increase client and provider program participation and collaboration. Outreach requires a degree of travel.
