Asbury Village is recruiting for a hospitality focused Health and Wellness Navigator to join our team! General Summary: The Health and Wellness Navigator works in accordance with the established policies and procedures of Asbury Village and/or specific instruction from the Health Services Director. The Health and Wellness Navigator works alongside health service team members to optimize health and wellness through the eight dimensions of wellness model. The Health and Wellness Navigator is responsible for providing health services to Asbury Village independent and assisted living residents by working with each resident individually to nurture psychological, physical, and social wellbeing, while simultaneously benefiting residents’ overall health. The Health and Wellness Navigator is responsible for building relationships, solving problems, locating resources, and being an integral part of the new resident acclamation program for independent living residents in conjunction with Health Services Director. The Health and Wellness Navigator is responsible providing home care services to designated residents. The Health and Wellness Navigator will assist Health Services Director in managing after hours on call. United Methodist Village fully embraces a culture of hospitality. Requirements

  • Licensed Practical Nurse with active unencumbered license.
  • Demonstrate excellent verbal & written communication skills/with efficiency in English, using proper grammar in a pleasant manner. Also, able to communicate effectively with both large & small groups.
  • Computer software skills, including Word, Excel, and data entry.
  • Must have the ability to work independently with self-discipline and self-motivation.
  • Strong interpersonal skills are essential.
  • Must have knowledge of and special sensitivity to the needs of the aging.
  • Must be able to concentrate with frequent interruptions.
  • Must be able to walk for up to 75% of the workday.
  • Must be able to walk throughout the entire building, up to one mile, occasionally pushing a wheelchair.
  • Must be able bend and stoop.
  • Must be able to lift and carry up to fifty (50) pounds independently

Responsibilities

  • Communicate with all members of the healthcare team on behalf of the residents.
  • Provide health and lifestyle options to support a resident’s desire to live their lives to the fullest extent desired.
  • Provide programs and affiliations to maximize opportunities in all areas of personal wellness utilizing the eight dimensions of wellness model.
  • Help residents and their families in obtaining health care referrals.
  • Provide emotional support and counseling, related to the clinical situation.
  • Directly involved with the development and enhancement processes with the aim to improve the clinical experience for referred residents or physicians.
  • Create a desirable place to live where a resident can choose to be engaged, fit, and connected to the world around them regardless of how the aging process affects them.
  • Communicates with referring physician offices and hospitals as required by the resident or physician.
  • Provision of care and services according to client’s home service plan.
  • Assists with new resident acclamation program by providing daily visits for first week of residency, providing facility tours, escorting to dining room and activities to promote connection.
  • Work with marketing and outreach departments to educate referring physicians, community organizations and facilities on services.
  • Assists Health Services Director in managing after hours on call.
  • Attends all in-service training and education sessions, as assigned.
  • Assumes responsibilities for personal work areas and maintaining areas in a presentable manner.
  • Performs specific work duties and responsibilities as assigned by administration.
  • Other duties as assigned.
  • Should be able to work in tough stressful conditions with a positive attitude.
