Charter Senior Living is seeking an experienced Health & Wellness Coordinator (also commonly known as an Assisted Living Nurse, Wellness Director, or Health Services Coordinator ) to oversee day-to-day healthcare operations within our senior living community. This role is ideal for a licensed LPN or RN who is passionate about resident care, clinical leadership, and enhancing quality of life for older adults. This role oversees resident health assessments, care coordination, and ongoing monitoring as healthcare needs evolve. The Health & Wellness Coordinator plays a vital leadership role in promoting resident dignity, regulatory compliance, and nursing best practices while supporting frontline care teams.

Requirements

  • Graduate of an accredited nursing program
  • Active LPN/LVN or RN license (required)
  • Must meet all state nursing and regulatory requirements

Nice To Haves

  • 1-2 years of nursing experience preferred (assisted living, long-term care, or senior living strongly preferred)

Responsibilities

  • Oversee day-to-day nursing and healthcare services in an assisted living environment
  • Coordinate resident care plans, assessments, and health monitoring
  • Ensure compliance with state regulations, assisted living standards, and company policies
  • Support medication management and clinical documentation processes
  • Collaborate with caregivers, CNAs, physicians, families, and leadership teams
  • Promote resident dignity, safety, and quality of life
  • Provide clinical guidance and leadership to care staff
  • Respond to changes in resident condition and coordinate appropriate interventions
