About The Position

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. Men’s Residential Step Down (MRSD) is a program of HealthRIGHT 360 located at Alamo Square neighborhood in San Francisco. It is a 64-bed male transitional housing for individuals that have completed at a minimum of 30 days residential substance use disorder treatment. MRSD provides temporary housing to assist our clients in their next stage of sobriety. Under the supervision of the Program Director, Medical Director, Behavioral Health Nurse Director, and/or Program Manager, the Health and Wellness Coordinator II splits their time between retrieving prescription medications from local retail pharmacies and fulfilling their core responsibilities in the role. They collaborate with providers to collect and manage protected health information (PHI) and are responsible for maintaining a secure inventory of medications and medication administration records. This role involves directly observing and documenting medication therapies for approximately 45 clients receiving treatment for substance use and co-occurring disorders. Additionally, the Health and Wellness Coordinator II provides health-related information within their scope of practice, serves as a resource to help clients navigate the healthcare system, and supports them in developing the skills to manage their own healthcare. They deliver services using Trauma-Informed and Harm Reduction approaches.

Requirements

  • High school diploma or equivalent.
  • EMT or Medical Assistant Certification, i.e., AAMA/CCBMA Certification.
  • First Aid certified within 30 days of employment.
  • CPR certified within 30 days of employment.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.

Nice To Haves

  • Two years of experience in a substance abuse treatment setting.
  • Course work in related subjects, i.e., co-occurring disorders, Trauma-Informed Care, Motivational Interviewing, HIV, HCV, Harm Reduction, etc.
  • Bachelor’s Degree in related field.

Responsibilities

  • Closely collaborates with Medication Assisted Therapy (MAT), Withdrawal Management, and other Health & Wellness teams to ensure treatment plans and medication regimens are accurately implemented on the unit.
  • They are responsible for the management of client medications, including psychotropic medications and controlled substances and associated documentation, secure storage, count/inventory, and destruction according to policy.
  • Acts as liaison with other staff and providers to relay requested information and reports on client adherence to medications.
  • Maintain and update all medication documentation, including:- Centrally Stored Medication and Destruction Logs- Medication Self-Administration Forms- Shift-to-Shift Controlled Substance Logs- Discharge/Transfer Medication Forms- Standard letters to physicians and agencies
  • Audit charts to ensure required medical documentation is complete and compliant with payor and regulatory requirements.
  • Perform intake, including vital signs, list chief complaints, allergies, immunization histories and complete chart forms, as appropriate to patient type.
  • Place patients in rooms and prepare them for examination.
  • Clean, prepare and stock examination rooms.
  • Receive and return telephone calls from patients and pharmacies.
  • Assist in scheduling specialty appointments and consultations.
  • Monitor and ensure quality assurance of basic clinic functions.
  • Oversee patient flow and ensure smooth, efficient patient flow throughout patient’s clinic experience.
  • Works collaboratively with all Health and Wellness staff.
  • Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the Director of Addiction Medicine.
  • Supports Medication Assisted Treatment through counseling, education, and administration of injectable medications under the supervision of the addiction provider and stand orders.
  • Provides appropriate client care, education and counseling.
  • Provides complex care and care coordination for all clients, accessing healthcare services internal and external to HealthRIGHT 360.
  • Works closely with stat collecting and reporting contractual data as necessary.
