About The Position

As a key member of the Residential Medical team, assist the Health and Wellness Practitioner (HWP) with all aspects of client medical care from admission through discharge. Ensure the continuity of physical and emotional care and safety for all residents, both internally and externally. Responsible for following all regulations and policies and procedures regarding the storage, administration, and distribution of client medications.

Requirements

  • High School diploma or equivalent
  • Prior experience in or sufficient knowledge of medical practice and care to assist in giving client care and supporting medical staff
  • Skilled in taking vital signs preferred
  • CBRF Certified, preferred
  • Basic knowledge of Substance Use and Mental Health Disorders
  • Employees who, as part of their regular duties, transport Meta House clients to and from meetings or appointments in their personal vehicle must carry and maintain proof of current Wisconsin automobile liability insurance coverage at or above the minimum state requirements.
  • Employees must present proof of insurance (insurance card or electronic equivalent) upon request by Meta House.
  • Driver’s license, safe driving record, willing to drive agency vehicles an insurable under Meta House’s insurance policy to drive agency vehicles
  • Effective written and verbal communication skills.
  • Strong organizational, time management, and planning skills.
  • Ability to collaborate and communicate with both internal team members and external agencies/partners.
  • Emotional intelligence and resilience when handling crises and long-term exposure to traumatic stories and experiences.

  • Computer skills including proficiency in Epic Connect, Microsoft Office Suite, and other applications
  • All Meta House staff are required to provide proof of vaccination for MMR, Varicella, and annual flu shot.

  • Able to: Tolerate occasional hot or cold environments.
  • Sit or stand for extended periods of time including alternating sitting or standing at will.
  • Lift and up to 25 pounds on a regular basis and 40 pounds on occasion.

  • Bend, stoop, twist, squat, and kneel to perform essential functions.
  • Reach overhead, and at or below shoulder level.
  • Read and enter text or data into a computer or other system.

  • Hear, understand, and distinguish speech and/or other sounds in order to communicate with clients and/or colleagues.
  • Express or exchange ideas by means of spoken word to impart oral information to clients or the public and convey detailed spoken instructions to others accurately, loudly, or quickly.
  • See clearly at approximately 20 inches or less including use of computers or tablet.
  • Visually identify clients and others for reasons including, but not limited to security, presence in the building, and medication dispensing
  • Valid Wisconsin driver’s license required.
  • Ability to operate agency vehicle for work related duties.

Responsibilities

  • Obtain and document needed clinical information at admission (vitals, urine screen, pregnancy test, TB test).
  • Support on-going medical appointment needs and medical care follow-up
  • Schedule medical appointments with clients both internal and external, in collaboration with Case Manager and transportation team
  • Work directly with clients on appointment reminders and life skills related to managing own appointments and follow up recommendations
  • Receive and process all incoming external documents (e.g. hospital records, lab results, clinic after-visit summaries, test results etc.); Request these documents as needed
  • Ensure all medical paperwork is scanned into chart and all medical charting is complete
  • Assist with the color of the day urinalysis process, including administrative duties and documentation, monitoring supplies and collection of urine and breathalyzer samples
  • Oversee compliance of Medication Management and Prescription policies and procedures
  • Monitor compliance related to medication management, administration, and disposal/destruction
  • Collaborate with leadership on any training or communication needs related to medication management
  • Contact and coordinate with pharmacies for refills, new/changed orders, and questions
  • Monitor and order medication refills
  • Organize medications for new admissions and prepares medications for client discharges
  • Maintain accuracy of information entered in ECP
  • Maintain medication cart and medication binders
  • Manage incoming phone calls, fax requests and emails to the medical team
  • Assist with the safe, timely, and organized transportation of clients
  • Perform all required record keeping and paperwork, accurately and timely

  • With supervisor, ensure coverage of all services, including group and individual sessions as assigned and coverage for colleagues' PTO time
  • Attend required meetings and clinical supervision
  • Participate in agency or community-based committees or meetings as assigned
  • Assist with other duties to support general program operations and occasional special programs
  • Other duties as may be periodically or permanently assigned
