Responsible for welcoming and orienting prospective patients to care management services and coordinating delivery of quality services through appropriate data gathering and resource referrals. Acts as the primary point of contact to facilitate effective patient communication in support of the care management team to ensure continuity of care across the care continuum and improve patient outcomes. This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:00 am - 5:00 pm PST. Our office is located at 5701 Katella Ave., Cypress, CA 90630. We offer 4 weeks of on-the-job training. The hours during training will be 8:00 am - 5:00 pm PST, Monday - Friday. (Day 1 onsite to receive equipment and then they will be remote) If you are located within a commutable distance to the office at 5701 Katella Ave., Cypress, CA 90630, you will have the flexibility to work remotely* as you take on some tough challenges.
