About The Position

Become a Home & Community Support Staff/PCA (Relief) in our Traumatic Brain Injury (TBI)/Nursing Home Transition & Diversion (NHTD) Home Care Support Services program and help others in your own community! Receive competitive pay and make your own schedule to fit the individual's needs and your needs. Multiple positions available in Chemung and Steuben counties. This position is responsible for the delivery of all HCSS activities as stated in the HCSS Plan. The HCSS/PCA must have excellent communication skills and must be capable of fostering relationships that value and support others. He or she must be a good role model in all settings and must be capable of utilizing opportunities to educate self and others. The HCSS/PCA must be able to perform personal care, light housekeeping, food preparation, and mobility tasks, as physician ordered for each participant. The HCSS/PCA must be a self-starter capable of functioning with minimal supervision.

Requirements

• High School diploma or GED preferred
• One-year experience in Human Services field preferred
• HHA or PCA certificate recommended
• COVID Vaccination or Valid Medical Exemption Required by NYS Department of Health

Responsibilities

• Delivery of all HCSS activities as stated in the HCSS Plan
• Perform personal care tasks as required
• Conduct light housekeeping duties
• Prepare food for participants
• Assist with mobility tasks as physician ordered
• Foster relationships that value and support others
• Educate self and others as opportunities arise
• Function with minimal supervision
