Become a Home & Community Support Staff/PCA (Relief) in our Traumatic Brain Injury (TBI)/Nursing Home Transition & Diversion (NHTD) Home Care Support Services program and help others in your own community! This position is responsible for the delivery of all HCSS activities as stated in the HCSS Plan. The HCSS/PCA must have excellent communication skills and must be capable of fostering relationships that value and support others. He or she must be a good role model in all settings and must be capable of utilizing opportunities to educate self and others. The HCSS/PCA must be able to perform personal care, light housekeeping, food preparation, and mobility tasks, as physician ordered for each participant. The HCSS/PCA must be a self-starter capable of functioning with minimal supervision.
