About The Position

Second Chances Comprehensive Services LLC is a caring organization with a focus on at-risk youth in the community. We provide mental health services in the Virginia communities around Hopewell, Virginia Beach, Chester and Emporia. Our founders recognized the need for strong community outreach services that may help facilitate and guide individuals with their future endeavors while maintaining family cohesiveness. The Qualified Mental Health Professional (QMHP) provides direct mental health services to adults, children, adolescents, and families in community-based, outpatient, and in-home settings. The QMHP works as part of a multidisciplinary treatment team to assess client needs, develop and implement treatment plans, provide therapeutic interventions, and support clients in achieving stability and improved functioning. Services are delivered in accordance with state regulations, agency policies, and evidence-based practices.

Requirements

  • Candidates must meet Virginia DBHDS QMHP eligibility criteria, including one of the following:
  • Bachelor’s degree in human services or a related field and at least 1 year of documented clinical experience providing direct mental health services; or
  • Bachelor’s degree in a non-human services field and at least 2 years of documented clinical experience providing direct mental health services; or
  • Master’s degree in a human services field (clinical experience may be required depending on role).

  • Strong understanding of mental health diagnoses, treatment modalities, and recovery-oriented care.
  • Ability to work with diverse populations, including children, adolescents, adults, and families.
  • Knowledge of trauma-informed care, person-centered planning, and cultural competency.
  • Effective verbal and written communication skills.
  • Strong organizational, time management, and documentation skills.
  • Ability to manage crisis situations calmly and professionally.
  • Proficiency with EHR systems and basic computer applications.
  • Ability to sit, stand, walk, and travel for extended periods.
  • Valid driver’s license, reliable transportation, and current auto insurance.
  • Ability to pass background checks, drug screening, and credentialing requirements.
  • Group supervision and Individual supervision is a MUST.

Nice To Haves

  • Previous experience in community-based mental health services (IIH, MHSS, TDT, outpatient, or group home settings).
  • Experience working with high-risk populations and co-occurring disorders.
  • Bilingual or multilingual skills is a PLUS

Responsibilities

  • Provide individual, family, and group mental health services under the supervision of a licensed clinician (as applicable).
  • Develop, implement, and monitor Individualized Service Plans (ISPs) and treatment plans in collaboration with clients, families, and treatment teams.
  • Deliver skill-building, psychoeducation, crisis intervention, and therapeutic support services.
  • Assist clients with coping strategies, emotional regulation, social skills, and daily functioning.
  • Provide crisis response and de-escalation interventions when clinically necessary.
  • Coordinate services with internal team members and external providers (schools, DSS, probation, medical providers, community resources).
  • Advocate for clients to ensure access to appropriate mental health, medical, educational, and social services.
  • Assist clients with referrals, linkage to community supports, and follow-up care.
  • Participate in treatment team meetings, case conferences, and staffing reviews.
  • Maintain accurate, timely, and compliant clinical documentation in accordance with Medicaid, DMAS, DBHDS, and agency standards.
  • Complete progress notes, assessments, treatment plans, discharge summaries, and incident reports within required timelines.
  • Ensure services meet medical necessity criteria and regulatory requirements.
  • Maintain client confidentiality in compliance with HIPAA and state regulations.
  • Work collaboratively with licensed supervisors, clinicians, and interdisciplinary team members.
  • Participate in clinical supervision, staff meetings, and required trainings.
  • Implement evidence-based and trauma-informed practices.
  • Maintain professional boundaries and ethical standards at all times.
  • Manage assigned caseload effectively and meet productivity expectations.
  • Utilize electronic health record (EHR) systems and agency software as required.
  • Support agency quality assurance, audits, and accreditation activities as needed.
