Under the general supervision of the Director of Home and Community Based Services and/or the PASSPORT Supervisor, the Case Manager (CM) is responsible for understanding and abiding by PASSPORT program contractual expectations. The PASSPORT CM is expected to manage and coordinate comprehensive services and assure that they are appropriate and meet the needs of the consumer to support them safely living in a Home and Community Based Setting. Must have the following abilities to perform the responsibilities: Ability to effectively communicate and build rapport with the public, in person, by phone, and via email, Ability to comprehend written material related to essential job functions, Ability to accurately document facts related to essential job functions, Ability to analyze, evaluate, and implement a reasonable course of action based on available information, Ability to lift/carry 20 lbs. upstairs, Ability to climb steps and access private homes/locations which may not be readily accessible. Ability to travel to community and consumer locations within our four-county service area (Ashtabula, Mahoning, Trumbull, and Columbiana counties), and The ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio. Maintains minimum proficiency in all requisite computer applications and software including entering data and managing in an excel spreadsheet. Participates in training to maintain and improve job skills.
