About The Position

The MCW (Medi-Cal Waiver) Program Coordinator will be responsible for fulfilling a broad range of analytical, planning, engagement, and implementation functions of the MCW which is focused on improving the health of most vulnerable patients. The MCW Program Coordinator, under the guidance of the Program Supervisor, will assist with program coordination during the startup phase through implementation of the MCW and will serve as a liaison for the care coordination work and activities being provided by the MCW staff across internal and external organizations. Primary duties shall include performing research and data analysis, reporting, operational and conceptual problem-solving, building partnerships, and coordinating patient care activities.

Requirements

  • Bachelor’s degree in health/ human services, marketing, public administration or related field, or active LVN license. Experience considered in lieu of education.
  • Valid CA driver’s license and access to an insured vehicle.
  • A minimum of 2 years of experience in health care or community-based organization setting performing case management, patient access, outreach, referrals or related function.
  • Superb organizational ability and exceptionally analytical.
  • Knowledge of local, state and federal public health assistance programs and respective policies and regulations (i.e. Medi-cal, Medicare, etc.). Proficient in Microsoft applications and use of electronic health data systems such as EPIC. At a minimum, have excellent computer/tablet literacy including internet navigation and research skill.
  • Good written and verbal communication.

Responsibilities

  • Create comprehensive project work plans and workflows that define the project purpose, scope, key milestones and tasks necessary to guide the implementation and maintenance of the MCW program initiatives.
  • Assist in monitoring MCW team’s progress and performance as it relates to the project activities and program requirements, ensuring proper documentation throughout the course of the program.
  • Assist in monitoring project performance using appropriate tools and techniques in order to fulfill quality measure reporting to the MCW-associated managed care plans.
  • Track and report on project status; identify issues and escalates relative issues in order to ensure timely completion of project tasks and milestones.
  • Serve as a liaison with internal and external partners in order to facilitate constructive dialogue addressing the needs of the MCW.
  • Assist in developing learning objectives, create materials for, and deliver educational presentations in small and large group settings to MCW staff as needed.
  • Support Case Managers as necessary to carry out direct care coordination duties with patients (e.g. educating patients on self-management skills, coordination/oversight of specialty care referrals, and facilitating linkages to social supports).
  • Establish and maintain direct communication with appropriate outsourced third parties to provide technical assistance, coordination, tracking, quality assurance, and support to ensure completion of MCW program initiatives by the said due dates.
  • Provides responsive, high quality service to patients, SYH employees, outside agencies, and members of the public by providing accurate, complete and up-to-date information in a courteous, efficient and timely manner.
  • Acts as a point of contact and communicate program status to appropriate Health Plans, including tracking and ensuring requests are completed in compliance with MCW requirements and by the said due dates.
  • Maintains established departmental policies and procedures, objectives quality assurance programs, safely, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Attends meetings as required and participates in committees as directed
  • Performs other related duties as assigned or requested.
