About The Position

Provides recovery support using a harm reduction approach to engage patients and link to care. Acts as a regional community and site liaison to expand substance use and primary care treatment access. Educates patients regarding MAT, HCV, risk reduction, medical and medication adherence. Offers training on MAT, Naloxone, and harm reduction resources. Connects patients to internal and external services.

Requirements

• Knowledge of stages of change, substance use disorders, services, and family dynamics.
• Understanding of common related medical and mental issues.
• Ability to engage patients and provide education on MAT and harm reduction.

Responsibilities

• Engage new patients to care and complete intakes.
• Demonstrate knowledge of harm reduction approaches to care and implement harm reduction practice into recovery support services provided to patients.
• Provide health education and adherence support to patients.
• Participate in ongoing training, supervision, and professional development opportunities.
• Facilitate presentations on Naloxone, substance use, harm reduction, and related topics.
• Provide referrals to recovery support services and community resources.
• Participate in clinical meetings, quality improvement, case conferences, staff meetings, and supervision.
• Complete recovery support plans.
• Demonstrate understanding of and compliance with all HIPPA, OASAS, and other health-related legal policies and procedures, as well as internal Sun River Health Center for Recovery policies and procedures.
• Act as a recovery support liaison for regional primary care sites providing education and referral resources.
• Ability to complete electronic and written forms and reports in an accurate and timely manner.
