About The Position

Mosaic is looking for a Habilitative Specialist/Case Manager to coordinate services. This role is described as a "Program Captain" who ensures services are delivered effectively and in compliance with necessary rules. The ideal candidate is a solutions-oriented problem solver with a thorough and conscientious approach, an efficient helper who can smooth bumpy situations, and a teacher who provides tangible solutions and helps others "connect the dots." The mission involves training staff in behavior modification techniques, developing new programs and strategies to help individuals learn and grow, and maintaining records of their success stories.

Requirements

• A Bachelor’s Degree in Psychology, Special Education, or a related Human Services field OR experience in lieu of the degree (5 years relevant work experience) would be needed.
• A minimum of one (1) year of experience working directly with individuals with developmental or intellectual disabilities is required.
• Proven ability to ensure program delivery is in compliance with regulatory requirements.

Nice To Haves

• Previous experience working within an Home and Community Based Waiver (HCBS) setting is a plus.
• Strong organizational skills, excellent communication, and a genuine desire to help others succeed.
