About The Position

GENERAL DESCRIPTION: Responsible for conceptualizing, developing, and monitoring person centered habilitation programs and behavior support plans that guide achievement of individual habilitation goals. ESSENTIAL JOB FUNCTIONS: (NOTE: Qualified persons with disabilities are encouraged to apply. Reasonable accommodation will be made.) Complete assessments to identify individual strengths, needs, safety risks, preferences, and abilities. Develop habilitation programs that reflect assessments and baselines and that are designed to achieve individual habilitation program goals. Develop behavior support plans, in consultation with the responsible clinician, that reflect behavioral assessments and that are designed to teach appropriate replacement behavior and decrease target behavior. Serve as Goodwill’s point of contact with clinicians providing the Consultative Assessment service. Develop safety plans that are designed to address identified behavioral risks and to ensure staff and providers are prepared to provide necessary supports. Remain on-site at base location as well as off-site locations (as needed) to train staff and providers on all programming, behavioral safety plans, and supporting documents as needed at implementation, when changes occur, and ongoing. Complete monthly service reviews to ensure progress. Modify habilitation programs, behavior support plans, and safety plans as needed. Serve as a member of the Individual Support Plan (ISP) team. Provide information to establish services, goals, and objectives to meet the individual’s needs and goals. Meet attendance and appearance requirements per Goodwill policies. Provide back-up for vacant positions as needed. Assure standards of safety for all employees and at all service sites. Perform work activities in conformance to Goodwill values (respect, integrity, compassion, stewardship, learning, teamwork and entrepreneurial spirit), immediately report any wrongdoing (unethical/improper conduct, fraud, fiscal mismanagement, etc.), embrace cultural diversity, and encourage/support others to do all the same. Other duties as assigned.

Requirements

  • Associates degree in human services or related field preferred.
  • Degree and/or 3 years minimum experience working with persons with intellectual and developmental disabilities.
  • Knowledge and experience in support processes such as assessments, habilitation planning, behavioral supports, person centered plans, and implementation/teaching staff of supports.
  • Good communication skills required.
  • Experience in data collection, documentation, and reporting required.
  • Must be at least 18 years of age.
  • Must have a valid driver’s license issued in the state of Nebraska and meets all insurance requirements.
  • Ability to communicate in spoken conversation, adequate volume speech, and have adequate hearing for conversation, telephone use and emergency information systems.
  • Ability to be independently mobile on all surfaces and situations, to move safely about the work area.
  • Visual acuity, depth perception and field of vision within normal range.
  • Physical stamina to be standing and safely lift objects up to 35 pounds.
  • Physical stamina to work at least eight (8) hours per day and be able to travel to conferences, meetings, service settings, etc.

  • Ability to utilize the Microsoft Office programs/other software to complete required documentation.
