About The Position

The Habilitation Specialist 1 position involves instructing and supporting individuals in developing and enhancing life skills necessary for independent living in home and community settings. The role includes developing, implementing, and modifying habilitation plans to help individuals achieve their valued outcomes. The specialist collaborates with individuals, family members, advocates, care managers, and service providers to determine strategies for service delivery, assesses individuals' skill levels, communicates with treatment team members, implements safeguards, monitors progress, and assists individuals in participating in community activities.

Requirements

  • A bachelor's or higher-level degree in a human services field.
  • Eligibility for 70.1 transfer by way of current permanent status in a title deemed comparable to Habilitation Specialist 1.

  • Knowledge of community resources and services for individuals with developmental disabilities.

Responsibilities

  • Collaborates with individuals, family members, advocates, care managers, and service providers to determine strategies for service delivery.
  • Attends Life Plan meetings to assess the effectiveness of habilitation plans and services and establish objectives.
  • Assesses individuals' skill levels in relation to their valued outcomes.
  • Identifies appropriate methods to teach and assist individuals in acquiring, improving, and maintaining life skills.
  • Communicates with treatment team members and gathers information to plan and/or modify services.
  • Implements appropriate safeguards and protective oversight measures to ensure the health and welfare of individuals.
  • Monitors, records, and measures individuals' progress in reaching their goals.
  • Modifies habilitation plans to ensure continual progress and meet individuals' needs.
  • Schedules, transports, and assists individuals in participating in community activities.
