About The Position
The Habilitation Specialist 1 position involves instructing and supporting individuals in developing and enhancing life skills necessary for independent living in home and community settings. The role includes developing, implementing, and modifying habilitation plans to help individuals achieve their valued outcomes. The specialist collaborates with individuals, family members, advocates, care managers, and service providers to determine strategies for service delivery, assesses individuals' skill levels, communicates with treatment team members, implements safeguards for health and welfare, monitors progress, and assists individuals in participating in community activities.
