The Habilitation Manager is responsible for the overall operation of the College Experience Program including: planning, coordinating and supervising services and staff to meet the individual needs of each person supported on the college campus and in the community. Essential Functions:Service Planning: Familiarize self with college campus and college experience curriculum. Be prepared to assist with delivery of curriculum and support all associated college activities. Be prepared to facilitate and lead classroom discussions, and provide tutorials to support individual participants’ learning styles and other needs. Accompany individuals served during class auditing and support material retention and use as applicable.Develop additional teaching materials to support unique learning needs, such as visual aids and modified supplementary notes Order or obtain materials needed for classes. Copy and distribute classroom materials. Work closely with Coordinator to review progress, issues or needs with assignments. Assist students to coordinate and attend regional and national conferences. Develop, write, update and implement person centered Staff Action Plans (SAP) and individual Safeguards Plan or Individual Plan of Protection (IPOP) for people supported.Ensure necessary level of supervision (LOS) is documented and implemented by staff. Ensure the individual’s Safeguard Plan, Individual Plan of Protection (IPOP), Behavior Support Plan (BSP), Plan of Nursing Services (PONS) dietary guidelines and any other applicable plans are available and followed.Implement Life Plan (LP) as developed by each person supported/family/advocate and Care Coordinator. Participate in program planning meetings at least semi-annually to coordinate services for people supported.Ensure that a pre-meeting and complete necessary assessments with individual team occur prior to all Life Plan meetings or semi-annual reviews. Write Monthly Summary of Individuals’ progress based on review of daily notes, progress notes, discussion with staff and direct observation/contact. Review and monitor the implementation of Staff Action Plans, IPOPs, and other services (as well as documentation of same) for accuracy, progress, and quality assurance purposes. As assigned, complete timely audit of daily notes for billing.Update Electronic Health Record (EHR) with current plans and other pertinent information for each individual supported.Assist program staff in developing and implementing daily activities for programs/room(s) assigned. Utilize a community-based curriculum to teach habilitation & life skills as necessary.When applicable, accurately document services contemporaneously in Electronic Health Record (EHR) or with paper note (as applicable) in accordance with the Agency, OPWDD, Medicaid and other funder requirements. Supervisory & Administrative: Interview, hire, train, supervise and evaluate necessary program staff.Train subordinates on service curricula and other aspects of the college program as applicable.Develop and communicate scheduling of staff and people supported.Maintain and submit program time sheets and mileage sheets.Provides direct service to individuals as needed and directed, including participating in community activities. Assist with monitoring and entering attendance information as needed. Ensure compliance with OPWDD and Medicaid regulations by providing necessary training to staff, implementing services and monitoring services.Complete incident reporting process as required for Program.Develop volunteer opportunities and ensure adequate support for people supported.Complete person-centered planning activities with people supported as necessary.Communicate with residential programs, families, outside agencies and community resources as needed to coordinate services. Communicate professionally with College faculty and staff and adheres to confidentiality standards. Maintain regular contact with people supported to ensure a good working knowledge of person’s needs, preferences and satisfaction with services.Maintain petty cash fund as needed and provide oversight to program purchasing. Monitor receipts and submit receipts in an organized and timely fashion.Serve as a role model for people support and represent the Agency positively in the community.Responsible for the general housekeeping of assigned areas. Complete all necessary trainings as assigned and ensure program staff complete necessary training.Adhere to Agency policies and procedures. Treat all protected health information consistent with HIPAA. Participate in corporate compliance and quality assurance activities as required. Assist with Agency safety programs and promote workplace safety.Conduct oneself in a manner consistent with the Agency Code of Conduct and Arc BrandOther related duties as assigned
