About The Position

Rutgers, The State University of New Jersey is seeking a Habilitation Counselor for teh Department of Child & Adolescent Services - Collaborative Behavioral Health Care Program (CBHP) within Rutgers University Behavioral Health Care. The purpose of the Habilitation Counselor position is to provide site and community based case management and supported employment services to individuals receiving treatment services through UBHC's Outpatient Clinic services.

Requirements

• Bachelor's Degree in a behavioral health or related human services field and one (1) year related experience.
• Applicants possessing a degree in Social Work must have an active New Jersey certification/licensure as required by the State Board of Social Work.
• Valid driver's license.

Responsibilities

• As part of the Collaborative Behavioral Health Care Program (CBHP) team, actively participates in hotline calls. Assures calls are handled in an effective and "consumer friendly" manner.
• Assesses and manages case management needs of youth and families and facilitates connection to Hub and community resources.
• Advocates for appropriate resources for clients and families based on assessed needs as measured by supervisory observation.
• Completes appropriate documentation and utilization review.
• Conducts professional and employee responsibilities in a manner that promotes CBHP program and UBHC objectives.
• Completes appropriate internal paperwork (paper or electronic) required by CBHP program, UBHC, and the University.
• Supports goals and objectives of University Behavioral Health Care.
