About The Position

Fraser is currently hiring a Program Coordinator/Designated House Coordinator to join our team. We are looking for a highly motivated and experienced individual with strong communication skills and a proven track record in coordinating services. Requirements

  • Candidates must qualify based on one of the following three as assigned by 245D, which includes a combination of work experience and education.
  • HS Diploma or Equivalent: four years of full-time work experience providing direct care services to persons with disabilities (adults or children) or nursing/assisted living home experience.
  • BA/BS in Human Services or related field with 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older.
  • Associate degree in Human Services or related field with 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older.
  • Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness and reflection.

Responsibilities

  • Manage staff coverage to meet the needs of the clients served.
  • Coordinate completion of all household repairs and other upkeep.
  • Support other Program Coordinators with on call duties as directed.
  • Coordinate the activities of daily living including personal care, hygiene, and grooming.
  • Provide direct cares to our clients and supervisory duties.
  • Collaborate with medical professionals to coordinate health and medical needs for each client served.
