About the position

The Group Facilitator at New Oakland Family Centers plays a vital role in delivering clinical care within the FACE to FACE Partial Hospital Program. This position involves leading therapeutic groups and collaborating with a multidisciplinary team to provide high-quality support and treatment to consumers in an acute setting. The facilitator is responsible for ensuring that all participants receive the necessary care to achieve optimal outcomes.

Responsibilities

• Lead therapeutic groups for consumers in the FACE to FACE Program.
• Collaborate with a multidisciplinary team to provide comprehensive clinical care.
• Implement treatment plans developed by Associate and Lead Therapists.
• Assist with discharge planning and coordination of care.
• Handle crisis situations as necessary, including physical intervention and CPR.
• Complete required documentation according to clinic policies and procedures.
• Attend and participate in daily administrative and clinical meetings.
• Fulfill all initial and ongoing training requirements.
• Perform other duties as assigned.

Requirements

• Bachelor's degree in social work, professional counseling, psychology, or a related field required.
• Mental health experience preferred.
• Ability to work well with diverse populations including children, adolescents, and adults.
