Hospice of the Piedmont (HOP) is seeking a skilled Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) who is passionate about psychosocial care, grief support, and interdisciplinary collaboration. Requirements

Current state licensure (LCSW or LPC).
Two years of social work or counseling experience in a social service setting, with at least one of the two years being in a healthcare setting.
Clinical experience with individuals, groups, and families.
Case management, plan of care coordination, and advanced care planning experience.
Knowledge of the process of death, dying, bereavement, and understanding of psychosocial factors operative in terminally ill pediatric and adult patients and families, required.
Requires a valid driver’s license, personal automobile, and current automobile liability insurance
Strong clinical documentation skills

Ability to coordinate care across teams
Ability to adapt to varied work environments and unpredictable schedules
Ability to apply core clinical counseling techniques in diverse clinical settings

Responsibilities

Provides psychosocial assessments, direct casework, and counseling services to pediatric patients and families.
Participates in the preparation, evaluation, and execution of the interdisciplinary care plan to ensure that the aims of continuity and comprehensiveness of care are accomplished
Works collaboratively as a member of the HOP Bereavement Team
Participates in Interdisciplinary Team meetings, staff meetings, and other meetings as requested
Prepares documentation for the clinical record per HOP standards and ensures all documentation accurately represents all services provided
Demonstrates understanding and adherence to NASW or ACA Code of Ethics and related organizational policies, including confidentiality regarding patients, families, staff, and the organization
Uses expertise in the emotional, social, and spiritual aspects of death and grief to support families and children. Identifies signs of complicated grief and high risk situations in families coping with a child’s end of life journey, as well as in children grieving the loss of a loved one.
Facilitates bereavement support groups for families and children
Participates in agency and community programs, including supervision, to strengthen skills and enhance performance.
Facilitates communication with outside agencies including Region 10 and Social Services.
Implements and plans for placement and discharges from facilities, hospitals, and hospice as well as level of care changes
Oversees advanced care planning, APS and volunteer referrals, funeral planning, and ensures accurate EMR documentation, including demographics, insurance details, and all discharge, placement, and level of care paperwork.
