The Graduate Nurse (GN), under the supervision of a Registered Nurse (RN), is responsible for assisting in the provision of quality care to residents at Live Oak Nursing and Rehabilitation Center. This role focuses on promoting the highest practicable physical, mental, and psychological well-being of each resident, guided by individual assessments and care plans. Key duties include observing, assessing, documenting, and reporting changes in resident conditions during rounds, ensuring the implementation of care plans, and administering prescribed medications. The GN also conducts rounds with physicians, records visits, performs narcotic reconciliation, identifies significant changes in resident conditions, and takes necessary actions according to facility policies. Additionally, the GN oversees and monitors aides, initiates and completes Incident and Accident Reports, and handles resident discharges. Communication with physicians regarding changes in resident condition requires completing a Nursing Assessment for Physician Calls beforehand, and the GN communicates with residents' points of contact for status updates or condition changes. The Director of Nurses and/or the Administrator may assign other special projects or functions.
