About The Position

The Graduate Nurse (GN), under the supervision of a Registered Nurse (RN), is responsible for assisting in the provision of quality care to residents at Live Oak Nursing and Rehabilitation Center. This role focuses on promoting the highest practicable physical, mental, and psychological well-being of each resident, guided by individual assessments and care plans. Key duties include observing, assessing, documenting, and reporting changes in resident conditions during rounds, ensuring the implementation of care plans, and administering prescribed medications. The GN also conducts rounds with physicians, records visits, performs narcotic reconciliation, identifies significant changes in resident conditions, and takes necessary actions according to facility policies. Additionally, the GN oversees and monitors aides, initiates and completes Incident and Accident Reports, and handles resident discharges. Communication with physicians regarding changes in resident condition requires completing a Nursing Assessment for Physician Calls beforehand, and the GN communicates with residents' points of contact for status updates or condition changes. The Director of Nurses and/or the Administrator may assign other special projects or functions.

Requirements

  • Has completed a RN program at an accredited vocational school or college
  • Must be working toward testing for a Registered Nurse license in the state of Texas

Responsibilities

  • Assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care
  • During rounds observes, assess, documents, and reports resident condition/changes

  • Administers medications as prescribed

  • Perform narcotic reconciliation with incoming and outgoing shifts
  • Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures
  • Oversees and monitors aides
  • Initiate and complete Incident and Accident Reports per facility policy
  • Discharge residents per facility policy
  • Complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition
  • Communicate with resident’s point of contact when they ask for a status update or if there is a change in condition
  • Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator
