Provide practical established nursing assistant skills to residents, following policies and procedures of this facility. Assist nurses and supervisory staff in the resident care on the unit. Providing quality care by checking resident and room to insure the care required is safe and done in a timely manner. Units must be kept neat and tidy. The nursing assistant must possess good communication skills with all levels of staff, as well as, visitors, family members and consultants. The nursing assistant will respect the resident confidentiality and support resident Bill of Rights. Attending in-service educational classes is a part of your regular assignment of duty and record of attendance will be kept. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Attend and participate in change of shift report and make walking rounds with off/on coming shift. Answer signal light and bells on a timely basis to determine the resident's need. Assist resident with bath functions (bed bath, tub or shower) as directed by the resident's preference and care plan. Assist resident with daily dental and mouth care (brushing teeth, dentures, oral hygiene, and special mouth care). Assist resident with dressing and undressing. Assist resident with nail care (clipping, trimming, and cleaning fingernails). Shave male residents. Keep hair on female resident's clean (facial, underarms, and legs) as desired by the resident. Assist residents with hair care (combing, brushing, shampooing, styling) when not provided by the beautician. Keep residents dry (change gown, clothing, linens) when wet or soiled; following correct procedures for the use of disposable incontinence products. Change bed linens per facility schedule and policy. Make beds - occupied and unoccupied. Make sure air mattresses are inflated. Turn and position bedfast residents every 2 hours or as otherwise indicated, maintaining proper body alignment and using correct positioning devices. Assist resident with bowel and bladder functions (taking to the bathroom, offering urinal, or bedpan). Maintain Intake and Output records as directed. Keep water pitchers clean and filled with ice water, straws and within easy reach when ordered. Check and report bowel movements and character of stool as instructed. Assist residents in preparing for activity and social programs. Check daily calendar of events and ensure that the resident is ready to participate when indicated. Transport residents to and from activities in the facility. Assist with lifting, turning, positioning, transferring residents into and out of bed, chairs, bathtubs, wheelchairs, lift, etc. Assist residents to walk using devices design to assist (canes, walkers, chairs, gait belts, etc.) as ordered. Weigh and record weights as assigned., measure and record temperature, pulse, blood pressure and respiration (TPR) Assist residents with eyeglasses and hearing aides. Answer call lights promptly and keep call bells within resident's reach. Assist with the care of the dying resident. Knock before entering the resident's room to ensure privacy of resident. Treat all residents fairly with kindness, dignity and respect and ensure all nursing care is provided respecting the privacy of the resident. Report all allegations of resident abuse and/or misappropriation of resident's property. Honor the resident's refusal of treatment request and report such requests to the Nurse/Patient Care Coordinator. Offer resident liquids to drink/hydration frequently throughout each shift, unless resident on fluid restriction Clean up any spills, spatters, messes (including BM and urine) made while providing ADL’s and completing toileting rounds SPECIAL RESIDENT CARE FUNCTION Observe and report the presence of pressure areas and skin breakdown. Provide preventative skin care by the application of skin barrier products to residents at risk. Report injuries of an unknown source, including skin tears. Provide daily indwelling catheter care. Provide daily and as needed care. Assist with the application of hand cones, splints, binders, slings and other devices. Provide daily Range of Motion Exercises. Report changes in resident's condition and behavior. Apply protective devices correctly and as ordered. Release protective devices and reposition resident every 2 hours. Use assistive and positioning devices to assure proper position such as pillows, cushions and wedges to prevent skin breakdown and contractures. Prepare residents for meals - take to the bathroom, wash hands, position in bed, take to the dining room, place bibs, etc. ALL NURSING ASSISTANTS MUST PARTICIPATE IN DISTRIBUTION OF TRAYS IN A TIMELY MANNER . Trays are served to all residents at the same table before moving to the next table. Trays are color coded for feeding level and ensure the resident is being served proper diet as ordered. Assist with feeding - cutting foods, encouraging, prompting, and pouring beverages, seasoning. Spoon-feed residents, sitting at the resident's level and SYRINGE FEED ONLY BY PHYSICIAN ORDER . Assist resident with identifying food on tray, offer substitutes and inform kitchen when they are needed. Record food and fluid intake and meal percentage on the flow sheets, reporting any changes in eating abilities to the Nurse. Follow dysphasia protocols as indicated for residents with difficulty swallowing. Perform after meal care - remove trays, clean residents with difficulty swallowing. Assist resident in consuming snacks and other nourishment. Ensure all linen carts empty – down laundry chute – by 7:30 a.m. and 11:00 a.m. as Laundry department unable to collect dirty linens from nursing units while meal carts on units Assure clothing protectors removed from residents directly after meal is complete and sent to laundry in a timely manner INFECTION CONTROL 1. Use proper hand washing technique and infection control measures. 2. Follow established isolation precautions and procedures. 3. Follow established procedures in the use of personal protective equipment. 4. Maintain a safe, clutter free environment. 5. Keep all poisons (medications, creams, lotions, cleaning solutions) locked and out of reach of the residents. 6. Keep resident's personal items out of the bath and tub rooms. 7. Keep floors dry, clean up spills immediately. 8. Before leaving work area store all equipment and supplies. 9. Follow established safety precautions in the performance of all duties. 10. Report all hazardous conditions and equipment to the Nurse/Patient Care Coordinator immediately. 11. Participate in fire and disaster drills. 12. Follow established smoking regulations and report all violations. 13. Report any communicable or infectious diseases to the Nurse/Patient Care Coordinator. 14. Operate equipment in a safe manner and report defective equipment to the Ward Clerk/Nurse.
