About The Position

Progressive Community Health Centers is hiring a full-time Geriatrician at their Lisbon Avenue Health Center in Milwaukee, WI. This position does not require working late nights, weekends, or holidays. Compensation is dependent upon experience and local market. Founded in 1999, Progressive Community Health Centers is an independent non-profit organization operating four clinic locations in Milwaukee, Wisconsin, with over 100 employees. Their mission is to improve community health and quality of life by providing culturally competent services. They offer high-quality, comprehensive care to approximately 16,000 patients annually, many of whom are low-income, and no one is turned away based on insurance status or ability to pay. The centers value a patient-centered philosophy and provide services including family medicine, internal medicine, women's health, urgent care, dental, radiology, integrated behavioral health, case management, health education, and benefits enrollment. The Geriatrician will provide and administer primary care physician services, focusing on care for older adults, in accordance with clinic policies and standards. This involves delivering outpatient care in a community-based setting to a diverse patient population, eliciting detailed patient histories, conducting complete physician examinations, reaching diagnoses, ordering appropriate laboratory tests and referrals, and counseling patients and families on health and disease.

Requirements

  • Must be a graduate from an accredited School of Medicine and Residency Program.
  • Eligible to be licensed to practice in the State of Wisconsin.
  • Have a thorough knowledge of the principles and practices of medicine commensurate with his/her training.
  • Board eligibility in Geriatrics is required.
  • Previous experience should be from a hospital or ambulatory clinic setting.
  • It is mandatory that all employees, employed providers and volunteers are vaccinated. All hired candidates must submit proof of vaccination prior to their orientation.

  • Board certification in Geriatrics preferred.

Responsibilities

  • Provide all physician care generally provided at an ambulatory care facility, with a focus on adults over the age of 65.
  • Responsible for the delivery of care, treatment, and referrals.
  • Arrange for referral resources for those medical specialties not provided at the clinic.
  • Evaluate patients, based on medical training and/or specialty, to determine the medical condition of the patient, through the performance of physical examinations, thereby establishing a diagnosis and proposed treatment plan.
  • Maintain high standard of medical practice and participate in the training development of the medical team.
  • Assist in medical education of both the patients and clinic staff.
  • As a member of the health care team, exemplify an example in attitude toward patients and staff, thereby performing duties with respect, dignity, privacy, and modesty to the patients.
  • Participate in the development of clinic protocols and formulary.
  • Make recommendations, suggestions, etc., on chart information forms.
  • Complete all documentation within 72 hours
  • Counsel patients regarding medical and other problems which might affect patient's health and well-being.
  • Perform such clinical procedures for patient's benefit which are within scope and skill of practitioner, and which are economically feasible.
  • Participate and cooperate in all reasonable efforts with the clinic to apply for and secure public or private grants, contracts, and donations, and all customary and desirable accreditations, licensing and permits.
  • Provide direction and leadership to the clinic's medical support staff, with regard to the administering of a patient's care.
  • Provide supervision in case assignments to physician assistant, including sign off on charts.
  • Perform other job-related duties, as may be assigned, by the Medical Director
