About The Position

Comunilife Inc. is looking for a full time Geriatric Social Worker for its new program in the Bronx, NY. The Geriatric Social Worker provides comprehensive assessments and coordination for older adult residents at the El Hogar residence. The social worker ensures residents receive the appropriate medical, mental health, and social supports to promote independence, health stability, and successful aging in place. This role focuses on maintaining residents quality of life through culturally sensitive, strength based interventions, linkage to community services and collaboration with on site staff.

Requirements

  • Masters Degree in Social Work required.
  • Two years of experience in the field of aging is preferred.
  • Knowledge of ESSHI or supportive housing programs
  • Direct service experience working with seniors and/or individuals with Medical, substance abuse, serious mental illness, and homelessness needs
  • 5 Day/35 work week Position (In-Office/Some Field Work)
  • Excellent oral and written communication skills
  • Proficient in Google Workspace program/Microsoft Office Suite.

  • Bilingual Spanish preferred.

Responsibilities

  • Conduct comprehensive biopsychosocial and geriatric assessments at intake and quarterly, focusing on the five domains of aging: physical health, mental health, financial stability, spirituality, and socialization.
  • Develop individualized service and wellness plans in collaboration with residents, family members, and the interdisciplinary team.
  • Provide supportive counseling, crisis intervention, and advocacy for residents managing chronic illnesses, mental health conditions, and end-of-life issues.
  • Conduct face-to-face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
  • Coordinate referrals and closed-loop follow-ups with community-based providers for primary care, specialty medical care, mental health, substance use, and home-care services.
  • Assist residents with applications and recertifications for entitlements, benefits, and insurance programs such as SSI/SSD, SNAP, EPIC, Medicare, Medicaid, and meals-on-Wheels.
  • Ensure that clients pay the balance of their rent, telephone and utility bills.
  • Ensure compliance with medication and wellness plans.
  • Identify residents for social, educational, and recreational engagement to reduce isolation and improve quality of life.
  • Document all assessments, service plans, progress notes, and referrals accurately and timely in AWARDS.
  • Participate in interdisciplinary meetings, case conferences, and monthly coordination meetings with staff.
  • Educate residents on independent living skills (budgeting, nutrition, safety, communication with neighbors, and community engagement).
  • Maintain confidentiality and adhere to ethical standards and cultural competency principles in all interactions.
  • When necessary and appropriate, accompany residents in medical appointments and/or arrange ambulate services.
  • Provide monthly caseload update to the Program Director.
  • Document all direct activities including resident served case closures and direct service advocacy interventions.
  • Maintain accurate case notes, service plans, and documentation in accordance with DOH ESSHI standards and program policies
  • Complete quarterly reporting per DOH ESSHI.
  • Participate in weekly supervision.
  • Submit a weekly schedule of visits.
  • Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors.
