About The Position

Join our PACE (Program of All-inclusive Care for the Elderly) in Western North Carolina. We provide comprehensive, team-based care for older adults so they can live safely and well in their communities.

Requirements

• Board eligible/certified in any specialty
• Experience treating complex geriatric, internal medicine, and mental health patients

Responsibilities

• Provide primary care for PACE participants including preventive, acute, and chronic disease management
• Collaborate with interdisciplinary team for care planning and oversight
