Position Highlights & Primary Responsibilities: Our ideal candidate will have a passion for the care of the older adult, prior skilled nursing and palliative care experience. Located in Central Massachusetts, practicing one of our premier partner skilled nursing facilities. 0.6 FTE/24 hours. Monday - Friday needed Provide value based care to patients on skilled rehab and long term care units. Partner with a physician colleague to manage patients based on goals of care: recovery of health, palliative or comfort care. Follow skilled therapy patients closely to track their progress and barriers. Lead multidisciplinary skilled rehab team including nursing, dietary, social work, PT, OT, SLP, and case management. Monitor and manage acute and chronic conditions common in nursing homes: CVA, progressive neurological conditions including Dementia (Alzheimer’s and other Dementia diagnoses), Parkinson’s Disease, MS, fractures, delirium and postoperative surgical patients, and end stage diseases. Longitudinal follow-up and care of long-term care patients residing at the facility per state regulations. Discuss goals of care with patients and families and caregivers. Requirements

  • National Board Certification
  • Licensed by the Commonwealth of Massachusetts board of registration of Nursing
  • Graduate of an Accredited Nurse Practitioner Program
  • Maintains BLS certification, professional certification as appropriate

Nice To Haves

  • Minimum two years of APC Experience
  • Geriatric or Palliative Care experience
  • Fluency in EMR systems and technology
