About The Position

You are invited to join our stellar Geriatric Medicine team at EvergreenHealth in Kirkland, WA. As a geriatric advance practitioner at EvergreenHealth, you will play a vital role in providing specialized medical care to our senior community. You will collaborate with a multidisciplinary team of healthcare professionals to ensure the well-being of residents while handling a range of services. The Geriatrician will diagnose and treat a broad range of medical conditions specific to the geriatric population, provide comprehensive care plans. EvergreenHealth Medical Group (EHMG) is a physician-led, physician-designed group of more than 350 EvergreenHealth primary care doctors and specialists who are employees of EvergreenHealth. EHMG includes 12 primary care practices, 48 specialty practices and teams of hospitalists and intensivists dedicated to in-patient care—all working side by side with you. Responsibilities

  • Conduct thorough medical assessments, diagnoses, and treatment plans for older adults
  • Focus on management of polypharmacy
  • Collaborate with the interdisciplinary team and specialists to develop and implement comprehensive care plans
  • Provide guidance and education to staff, patients, and their families on geriatric care
  • Participate in care conferences and interdisciplinary meetings
