About The Position
Nuvance Health, now proudly part of Northwell Health, is seeking a full-time Geriatric Medicine Physician to join our dynamic and mission-driven team in Rhinebeck, NY. This is a unique opportunity to shape the future of geriatric care while mentoring the next generation of physicians. As a Geriatric Medicine Physician, you will: Provide comprehensive, person-centered care to older adults, including those with multiple chronic conditions. Lead a Geriatric Inpatient Consult Service at Northern Dutchess Hospital. Establish a patient panel at local long-term care facilities. Provide Consultative Geriatric Outpatient Assessments in a flourishing in-demand practice. Serve as faculty for our Geriatric Medicine Fellowship, precepting fellows and Family Medicine residents in hospital and rehabilitation settings. Collaborate with an interdisciplinary team including social workers, behavioral health specialists, and a large multi-specialty medical group. Opportunities for faculty development and career growth.
