About The Position

RVNAhealth is seeking a compassionate and experienced Geriatric Care Manager to play a key role in the development and delivery of our new Geriatric Care Management Program. This position is ideal for a MSW, LCSW, CGCM, or an RN/LPN with a CGCM certification and who is passionate about supporting seniors and their families through comprehensive, person-centered care-right in the comfort of their own homes. As a Geriatric Care Manager, you will work directly with older adults and their caregivers, providing psychosocial assessments, care coordination, advocacy, and ongoing support. You will collaborate closely with an interdisciplinary healthcare team to develop and implement care plans that promote independence, dignity, and overall well-being. Hours: Full Time (37.5 hours/week) - Flexible schedule based on client needs Location: This is a Field-based position covering towns in Fairfield County.

Requirements

  • Must have one of the following licenses/certifications: Master of Social Work (MSW) Licensed Clinical Social Worker (LCSW) in CT Certified Geriatric Care Manager (CGCM) RN or LPN License AND Certified Geriatric Care Manager (CGCM)
  • Minimum of two (2) years of experience working with geriatric clients and dementia care.
  • Experience with aging-related issues including caregiving, chronic illness, and end-of-life care
  • Certification in care management through NACCM or ability to obtain within six (6) months of hire
  • Valid driver's license with ability to travel in all weather and environmental conditions

Responsibilities

  • Assist in the development and implementation of the Geriatric Care Management Program
  • Participate in interdisciplinary case conferences and contribute to comprehensive patient care plans
  • Coordinate medical services and identify needs for additional home health services
  • Refer clients and families to appropriate community resources and services
  • Conduct psychosocial assessments to identify client strengths, needs, and preferences
  • Develop individualized care plans addressing physical, emotional, social, and environmental needs
  • Provide counseling, education, and ongoing support to clients and caregivers
  • Advocate for client rights, preferences, and quality of life
  • Provide caregiver stress relief and guidance
  • Participate in agency-wide community presentations
  • Attend community events and establish relationships with partners and referral sources
  • Promote awareness of StayingWELL and RVNAhealth programs and services
  • Maintain accurate, timely documentation of all client interactions and interventions
  • Remain knowledgeable and compliant with all agency, regulatory, and legal requirements, including HIPAA, DOH, and Corporate Compliance standards
  • Stay current with best practices in geriatric social work and care management
