About The Position

RVNAhealth is seeking an experienced and compassionate Geriatric Care Manager for our Geriatric Care Management Program supporting older adults and their families across Fairfield County. As a Geriatric Care Manager, you will work directly with older adults and their families to assess needs, coordinate services, and develop personalized care plans that support safety, independence, and quality of life. You will serve as a trusted advisor and advocate, helping families navigate healthcare systems, community resources, and long-term care decisions.

Requirements

  • Master of Social Work (MSW)
  • Licensed Clinical Social Worker (LCSW) – Connecticut
  • Certified Geriatric Care Manager (CGCM)
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) with CGCM certification
  • Minimum 2 years of experience working with older adults
  • Experience with dementia care, caregiving support, or chronic illness management
  • Background in care coordination, case management, or community-based services
  • Certification through NACCM (or ability to obtain within six months of hire)
  • Valid driver's license and ability to travel throughout Fairfield County, CT
  • Strong communication, advocacy, and relationship-building skills

Responsibilities

  • Conduct comprehensive psychosocial and care assessments in clients' homes
  • Develop individualized care plans addressing medical, emotional, and social needs
  • Coordinate healthcare services and community supports
  • Provide guidance and counseling to families navigating caregiving challenges
  • Advocate for client preferences and quality of life
  • Support families dealing with dementia, chronic illness, and aging transitions
  • Collaborate with physicians, home health clinicians, and community partners
  • Participate in interdisciplinary care conferences and program development
