About The Position

Join CentraCare as a Geriatric Behavioral Health Unit Program Facilitator, where you will play a key role in designing, planning, and leading therapeutic programming for patients in our inpatient geriatric behavioral health unit. This includes delivering education groups, activity groups, and therapeutic interventions that support the emotional, cognitive, and social well‑being of older adults. Working closely with the interdisciplinary team- you will help develop individualized Plans of Care, participate in team meetings, and communicate patient progress and performance effectively. You will document therapeutic interventions, patient engagement, and goal progression using individualized care plans, individual treatment plans, progress notes, and flow sheets. In this role, you will also provide day‑to‑day operational supervision, instruction, and support to Mental Health Associates (MHAs) to ensure consistent, high‑quality delivery of programming. Additional responsibilities include ordering supplies and educational materials, maintaining an organized and safe therapeutic environment, and contributing to Performance Improvement initiatives by offering recommendations that enhance patient care and program operations.

Requirements

  • Bachelor’s degree in Occupational Therapy (OT) with relevant experience in developing activity programs for geriatric behavior health patients OR
  • Bachelor’s degree in Therapeutic Recreation with relevant experience in developing activity programs for geriatric behavior health patients OR
  • Bachelor’s degree in Psychology with relevant experience in developing activity programs for geriatric behavior health patients.
  • Good verbal and written communications skills required.
