The Nurse Practitioner is accountable to facilitate a quality hospital experience and outcome for patients within a cost-effective time frame. The Nurse Practitioner will be responsible to the Medical Director of Patient Services and responsible for overseeing the progress of a specific caseload of patients across the care continuum to include out-patient clinics, specialty clinics, pre-admission and post-discharge when feasible. Acts as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum. Responsibilities: Evaluating patients in Genetic practice- in clinic and in-patient hospital setting. Coordinating patients' care, treatments and testing. Ordering appropriate tests and/or medications as needed May also include managing emergencies- both on site and via phone, telephone triage and supportive care. Work closely with the team of 6 genetic counselors and 2 metabolic dieticians One year of experience strongly preferred Experience managing individuals with genetic conditions including high-risk surveillance, or complex patients preferred. Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics. Provides direct care to patients in acute care, ambulatory care and home care settings. Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee. Participates in the education, clinical research and mentoring of healthcare providers. Facilitates patient/family education in area of expertise. Annually completes mandatory safety infection control, BLS, and ACLS/PALS. Prescriptive privileges per licensure.
